Donation Receipt Template Ready for Immediate Use

This donation receipt template helps charities, nonprofits, and other recipients formally acknowledge a gift of money or property. It provides space for donor and recipient details, the donated amount or item description, the date received, and a statement confirming whether goods or services were provided in return.

A donation receipt is a written acknowledgment that a person or organization has made a gift to a recipient. It helps preserve an accurate record of the donation, identify the parties involved, and clarify whether the donor received anything in return. A well-prepared receipt can support internal accounting and, where applicable, a donor's tax records. This ready-to-use model can be adapted for monetary donations, goods, or other property.

What a donation receipt is for

The receipt documents that a contribution was received on a specific date and identifies the donor, the recipient, and the nature of the gift. It is commonly used by charities, foundations, community groups, schools, religious organizations, and other entities that accept voluntary contributions.

Its purpose is evidential rather than promotional: the information should be accurate, complete, and consistent with the recipient's financial records. Tax deductibility depends on the law applicable to the donor and recipient, as well as the recipient's legal status.

Information to include

A clear receipt should contain enough information for the parties to identify the donation without relying on separate documents. The table below summarizes common fields and the mistakes that should be avoided.

Document fieldPurposeFrequent error
Receipt numberAllows the recipient to track the record internally.Using duplicate or missing reference numbers.
Donor name and addressIdentifies the person or entity making the gift.Using an informal name that does not match records.
Donation dateEstablishes when the contribution was received.Recording the preparation date instead of receipt date.
Amount or property descriptionStates what was donated and its relevant details.Listing vague descriptions such as “items.”
Return benefit statementClarifies whether goods or services were provided to the donor.Omitting benefits received in exchange for the gift.

Cash and electronic gifts

For a cash, check, card, or bank-transfer contribution, state the exact amount, currency, and payment method where helpful. The receipt should not state that a donation is tax-deductible unless that statement is legally accurate in the relevant jurisdiction.

Donated property and in-kind gifts

For goods or property, describe the items precisely, including quantity, condition, serial number, or other identifying details when relevant. In many jurisdictions, the recipient should acknowledge what it received but should not assign a value unless authorized or required to do so.

How to complete the template

Complete the receipt as soon as reasonably possible after accepting the donation. Keep a copy with supporting records, such as payment confirmations, inventory entries, correspondence, and authorization documents.

  1. Assign a unique receipt number and enter the date and place of issue.
  2. Write the recipient's legal name, address, and registration or tax identifier if applicable.
  3. Enter the donor's full name and contact details.
  4. Describe the donation accurately, add any benefit statement, and sign on behalf of the recipient.

Good recordkeeping practices

Use a consistent numbering system and keep receipts in chronological order. If a correction is necessary, issue a revised receipt that references the original record instead of altering an already delivered copy without explanation.

  • Verify the donor's name before issuing the receipt.
  • State the donation date in an unambiguous format.
  • Keep proof of payment or delivery with the receipt copy.
  • Describe non-cash gifts in sufficient detail for identification.
  • Retain records for the period required by applicable law and internal policy.
Do not promise tax treatment on a receipt unless the recipient's status and the governing tax rules have been verified.

Common issues to avoid

A receipt should not resemble an invoice, since a donation is generally voluntary and not payment for a commercial sale. If the donor receives admission, merchandise, meals, advertising, or another benefit, identify that benefit and follow the disclosure requirements that apply in the relevant jurisdiction.

It is also important to distinguish an acknowledgment of receipt from a valuation or appraisal. Independent valuation may be required for certain non-cash contributions, and the recipient should avoid making unsupported value statements.

Frequently asked questions

Can an individual issue a donation receipt?

An individual can acknowledge receiving a gift, but whether the donor may claim any tax benefit depends on local law and the recipient's legal or tax status. The receipt should not make claims that cannot be supported.

Is a signature required?

A signature is strongly recommended because it identifies the person authorized to acknowledge the donation. Electronic signatures may be acceptable where permitted by law and organizational practice.

What if the donor receives a benefit?

The receipt should state the nature and, where required, the value of the benefit received. Only the portion treated as a qualifying donation under applicable law may be eligible for tax treatment.
